Common questions

Can THREM cure prostatitis?

No treatment has been shown to “cure” CP/CPPS, and THREM is no exception. The published 20-person retrospective study observed lower symptom scores at 6-month follow-up, with 17 of 20 completers self-rating at least 60% relief of their main symptoms — but it had no control group and a small sample, so it cannot predict individual outcomes. For a chronic condition, measurable improvement in symptoms and quality of life is the realistic goal.

How long is the course?

The published protocol was once daily for 7 consecutive days; the clinic currently schedules 7–10 days, each session including a herbal retention enema and perineal massage. The exact number of sessions and your suitability are decided after an in-person visit. Responses vary widely and we do not promise how quickly effects appear.

Will symptoms come back after treatment?

The published study followed patients only to 6 months, so there are no long-term recurrence data and we cannot quote a recurrence rate. CP/CPPS tends to fluctuate and recur; after any treatment, continue to attend to pelvic floor function, sleep, mood and lifestyle, and see a clinician promptly if symptoms change.

Is it suitable for older patients or those with BPH?

The published study excluded men over 60 and those with BPH or prostate cancer, so current evidence cannot be applied directly to these groups. Urinary symptoms in middle-aged and older men are often related to prostate enlargement or other conditions; complete a local urological evaluation first. Suitability for THREM is decided by the doctor after an in-person visit.

What does treatment involve — is it painful?

THREM involves rectal administration and hands-on work in the perineal area. Sensations vary and local discomfort can occur. At the in-person visit the doctor explains each step, possible adverse effects, how to stop at any time and how privacy is protected, and written informed consent is obtained. We do not promote the treatment as “painless” or “comfortable”.

When should I seek care immediately instead of waiting to consult?

Fever, chills, blood in the urine, acute voiding difficulty or inability to urinate, or rapidly worsening pain all require immediate local emergency or urological care. This site does not provide emergency services or online diagnosis.
